Please print and fill out the form and include it with your order. To tally your order, please use the "Purchase Print" buttons on the website. These will open up a separate window in which your order will be caculated. Please put all of the information on that page onto the mail order form, including shipping costs, and send the TOTAL amount in the form of a check or money order to the address located on the upper right hand corner of the form.
